Transition planning is a process for students with an Individualized Education Program (IEP) to decide what they want to do after high school. In Illinois, transition planning starts at age 14 and a half. Join Family Matters PTIC as we help students understand what their transition plan does for them and ways to advocate for the appropriate plan
The target audience are youth ages 14 to 22.
