Person-Centered Transition Planning
Person-Centered Transition Planning Using the Charting the LifeCourse Framework
This professional development training, offered in partnership with IESE, focuses on strengthening person-centered transition planning for students with disabilities using the Charting the LifeCourse (CtLC) framework. Designed for Illinois educators and related service providers, the session connects CtLC principles and tools to Illinois-specific transition requirements, including Indicator 13, age-appropriate transition assessments, and coordinated transition services beginning no later than age 14½.
Participants will learn practical strategies to elevate student and family voice, develop meaningful postsecondary goals, and align IEP transition components with real-life outcomes. Attendees will leave with tools and examples that support both compliance and high-quality transition planning.
